We are facing a tremendously surreal amateur game.
On the first screen, there are some objects to interact; by clicking on them they'll comment some things.
One of those keystrokes will take you to another part of the game, where you are a "sperm?" and you have to dodge various obstacles.
The author himself says in the manual the folly of development and the need to discover for oneself the object of the game.
